Admissions Procedure by Department

Outpatient Rehab Therapy

1. Ask your physician to write a prescription for you to Levi Hospital.

2. You may bring your prescription with you or have your physician fax it to 501-622-3331 for the Levi Prospect Campus or to 501-623-7654 for the Levi Satellite Clinic at the YMCA.

3. Call 501-622-3412 for the Levi Prospect Campus or 501-622-3336 to schedule your first appointment.

4. For Levi Prospect Campus appointments, please arrive at the Admissions Office (300 Prospect Avenue - 3^rd Floor – Drive around the back of the hospital for 3^rd Floor entrance – Admissions is the first door on the left) 15 – 30 minutes before your appointment to fill out patient paperwork.

5. After filling out the paperwork at Admissions, you will go down the hall to check-in at the Outpatient Rehab Therapy office.  You will then be escorted to your actual therapy appointment.

6. For appointments at the Levi Satellite Clinic at the YMCA, you will walk into the YMCA (130 Werner Street) and check in at the Outpatient Therapy window on the right as soon as you walk through the door.  Please arrive 15 – 30 minutes before your appointment to fill out patient paperwork.

Transitions – Intensive Outpatient Therapy

1. Call 501-622-3587 to talk to the Social Worker and make an appointment.

2. Transitions is located at 300 Prospect Avenue on the 5^th Floor.

Inpatient Adult Psychiatric Program

1. Call 501-622-3520 if you would like to have an assessment for your condition.  If you feel like you will be of harm to yourself or to others, please call 501-622-3520 for help.  We are here to help you.

2. You can also ask your physician to refer you to Levi Hospital, but if you need IMMEDIATE help, please call 501-622-3520!
